Health Screening

Anyone planning to visit a Baker College, Culinary Institute of Michigan or Auto Diesel Institute of Michigan campus must complete the appropriate COVID-19 Self-Certification Health Screening below prior to arriving on campus.

Please review the Self-Certification Health Screening Protocol document for more information.

• Baker College Employees and Students: Click here to login and then select the COVID-19 Health Screening Form. This form must be completed daily prior to coming to campus.
• Baker College Guests/Visitors: Please complete the BC Visitor Health Screening Form.
